Eligibility & Requirements

To be eligible for a Starfish Scholarship the young lady must be clinical obese as determined by a BMI (body mass index) >30 and eligible for the high school grades of 9-12. 

Requirements to apply:

• a history of volunteerism or service work in the student's home community. 
• 2 letters of recommendation (can be from clergy, volunteer supervisor, teacher, non-parent family member) 
• Personal statement (from the student). How has weight effected your life. What do you think has contributed to your struggle with obesity? Who are you committed to help through your own success either in health or other areas? 
• Personal Statement (from Parent(s). How has your child been affected by their struggle with weight. What is your commitment to supporting them making the changes necessary?
• Letter from the primary care physician/ pediatrician recommending a weight loss program. 
•  A copy of the most recent tax returns to verify financial need.
